As a nonprofit, 501c3 organization, we depend upon the financial support of the communities we serve. There are several ways you can become a donor and support the mission of Community Health Professionals.
Support a Specific Program
You may also specify the program that you wish your donation to go toward. These include:

Patient Care Fund - As a nonprofit organization, Community Health Professionals does not deny care to anyone; regardless of ability to pay. Your donation will help us fund care for those who lack medical coverage or financial means to pay for home health care.

In-Home Hospice - Your donation supports hospice care provided in the home. There are many costs incurred in hospice care that are not adequately funded by Medicare, Medicaid or insurance. Your donation will help us fund those expenses and provide care for those who lack medical coverage or financial means to pay for hospice care at home.

Inpatient Hospice - Your donation helps fund general operations and patient care at our inpatient hospice centers. Several naming opportunities are also available; contact us to learn more.

Beacon of Hope - fundraising event to support your local hospice program

Memory Bricks & Leaves - Y ou can extend a lasting tribute in memory of a loved one who has been served by hospice or simply have your name inscribed as a donor to either the Van Wert or Defiance Inpatient Hospice Center. Memorials are not limited to inpatient hospice patients -- you may purchase a brick in memory of anyone you wish. You can make a donation to
ANY CHP program in memory of a loved one.
If a program is not listed in the list of programs on the donation page, type in the program after the person's name you are giving in memory of.
