Inpatient Hospice Centers

 
When a life-limiting illness is no longer manageable at home or an able caregiver is not available, CHP's Inpatient Hospice Centers in Defiance and Van Wert provid e a compas sionate alternative.  These free-standing residential facilities allow patients to remain in a homelike setting.

Levels of Care 
Levels of care are determined by the needs and conditions of the patient:
  
  • Acute Care - Under Medicare Hospice regulations, General Inpatient Care is only for uncontrolled symptoms or complications. The staff develops a plan to help both patient and family.
  • Residential Care - If a patient no longer meets the criteria for General Inpatient Care, he or she is switched to Residential Care. If a patient who has been switched to Residential Care develops uncontrolled symptoms or complications, he or she is returned to General Inpatient Care. The key is the patient's daily medical condition.
  • Respite Care - Medicare allows Respite Care under specific circumstances to provide reli ef for a family caregiver. Medicare covers a hospice patient who leaves his or her home and enters inpa tient care for up to five days.

CHP's inpatient hospice centers feature amenities to help families to be comfortable during their loved ones stay:

  • Internet access
  • Pet visitation
  • Sleeping accommodations for family
  • Unrestricted visiting hours

Highly Trained Staff
The inpatient hospice staff includes nurses who specialize in palliative, or comfort care; hospice-trained health aides; licensed social worker to assist with financial, legal and emotional needs; licensed massage therapist; chaplain for spiritual needs; and trained volunteers to assist families with various needs.


Admission Criteria
Enrollment in hospice is required, however it does not have to be established prior to admission. In such cases, enrollment is incorporated into the admission process.

In addition to meeting general criteria, specific guidelines include, but are not limited to:

  • Management of uncontrolled symptoms
  • Persons without family caregivers
  • Caregivers that need respite relief
Payment is re ceived from many sources, and depends on the level of care. CHP provides care to all, regardless of age, gender, race, religion, disability, diagnosis or ability to pay.

 
Memory Brick Program
The Memory Brick program offers you the chance to extend a everlasting tribute in memory of a loved one, to honor families or individuals who have been served or touched by hospice or simply have your name inscribed as a donation to a CHP Inpatient Center.
Your tax-deductible gift will purchase a brick or paver that will become a permanent part of a walkway at either the Defiance or Van Wert Area Inpatient Hospice Center.
