Home can and should be a special healing place...familiar and comfortable. With hospital stays so short today, home care often involves much more than simple bed rest. Visiting Nurses provide a complete range of services for clients of all ages with a variety of needs.

Skilled nursing care includes services ranging from basic nursing care to more intricate procedures like infusion therapy, wound care, PICC lines, ventilator care and psychiatric mental health nursing.  Medical Social Services, and Home Health Aide services are also available.

Community Health Professionals offers collaborative psychiatric mental health nursing services to patients in their homes. Services are provided by a qualified nurse with experience in psychiatric nursing and are designed to: prevent hospitalizations, increase access to mental health services, provide follow-up treatment, and assist families in maintaining community living and improving their quality of life. Health Monitoring: CHP is a partner with Honeywell HomMed, the leader in telemonitoring technology. With CHP and HomMed, it is now possible for home care clients to monitor their own vital statistics and transmit data via phoneline or wireless digital paging. Results are tracked and monitored by a professional clinician on a daily basis with important, accurate daily information passed along to client physicians.
