CHP's Adult Day Care Centers in Archbold, Celina and Van Wert offer a flexible and unique daytime alternative to nursing home and homecare services for aging and disabled loved ones who want to stay home.

Our program offers stimulating activities that add variety and interest to every new day while providing nursing care and attention in a home-like environment. The enjoyable atmosphere and loving care promote health and independence.

As a medical model facility with nursing supervision, CHP's Adult Day Care Centers provides an integrated program designed to address physical, psychological, cognitive and functional capabilities. The goal is to enable each individual to remain in his or her surroundings, maintaining the highest levels of dignity, independence, health and safety possible.

At the same time, Adult Day Care Centers provide needed periods of respite for family caregivers, enabling them to continue or resume employment, and tend to the necessary activities of their lives.

CHP's Adult Day Care Centers offer specialized programs for those suffering from Parkinsons disease, stroke, Alzheimers disease, diabetes and many other conditions. Adult day care is NOT a nursing home or assisted living facility. Clients stay at the center for a few hours at a time, or for the entire day, before returning to their own home each evening.

Who is Eligible?

• Those age 18 and older
• Has a primary Care physician
• Has physical or mental impairment due to illness,disease or normal aging
• Is not aggressive or abusive to others
• Needs intermittent supervision
• Is able to walk or transfer with assistance from
  wheelchair

What We Provide

• Assistance with activities of daily living including bathing, hair care, fingernail and toenail care, incontinence care and more
  
• Stimulating activities including individual and group exercise, recreational activities, mind-memory exercises, music and crafts
  
• Monitoring, supervision and coordination of physical, occupational and speech therapies as requested by a physician
  
• Nutritious noon meal and two snacks approved by a licensed dietitian
  
• Quiet areas available for resting or reading
  
• Spacious living area with room for walking and movement

Levels of Care

• Basic Care
  For individuals needing socialization, reassurance,
  minimal supervision and minimal assistance with ADL
  (Activities of Daily Living)
  
• Enhanced Services
  For clients requiring moderate assistance with ADLs,
  functional maintenance, therapy services and/or
  assessment and monitoring by the nursing staff
  
• Intensive Services
  For individuals needing maximum assistance, including
  those with Alzheimers disease, we provide assistance and
  support. Rehabilitative or restorative therapy is also
  available
  
  

Transportation is available or may be provided by your local council on aging or senior center. Nursing and therapy services including physical, occupational, speech and massage therapy are available as ordered by a physician.
