[pal-ee-uh-tiv] adj. - Relieving or soothing the symptoms of a disease or disorder without effecting a cure.

How is Palliative Care Provided?
Community Health Professionals home health and hospice team of registered nurses, nurse practitioners, doctors, home health aides, social workers, massage therapists, musician, chaplains and trained volunteers work together to provide treatments and soothing therapies to help relieve physical pain and other irritable symptoms.
They also provide emotional and spiritual counseling to help patients cope with their feelings so they can have internal peace and comfort. Palliative care is provided wherever patients reside, including patient homes, nursing and assisted living facilities or at our inpatient hospice centers.
CHP Hospice - Palliative Care Services
- Visits from registered nurses, nurse practitioners, home health aides, and trained volunteers
- Counseling with social workers and chaplains for patients and family members
- Palliative arts such as massage therapy and music ministry
- Help with accessing community resources and financial assistance resources
